ABSTRACT

Objective:To investigate the relationship between sarcopenia and the maximum diaphragm excursion(Dmax)observed on ultrasound in the elderly. Methods:Elderly volunteers(age≥60 years)were recruited from family members of patients at Guangdong Provincial People's Hospital.Their Dmax during forced inhalation was measured via ultrasound. The parameters for the diagnosis of sarcopenia included the appendicular skeletal muscle mass index(ASMI), handgrip strength and usual gait speed. We compared the differences in physical characteristics, pulmonary ventilation, physical performance and Dmax between patients with and without sarcopenia, and evaluated the relationship between sarcopenia and DEmax in the elderly via linear regression. Results:A total of 145 elderly volunteers[age(69.47±5.15)years]were included, and 28(19.31%)were diagnosed with sarcopenia.Body weight, ASMI, maximum inspiratory pressure(Pinmax), maximal power output(Wmax)and Dmax of patients with sarcopenia were significantly lower than those of patients without sarcopenia(all P<0.05).Dmax in the elderly was correlated with sex, height, ASMI, handgrip strength, usual gait speed, Pinmax and Wmax( r=0.181, 0.130, 0.322, 0.373, 0.401, 0.134, and 0.388, P=0.012, 0.037, 0.009, 0.002, 0.022, 0.009, and 0.002, respectively). After adjusting for sex, age, height and forced vital capacity(FVC), there was still a negative correlation between sarcopenia and Dmax in the elderly( β=-0.310, P=0.021). Conclusions:Dmax is related to Pinmax and physical performance in the elderly, and sarcopenia increases the risk of decline in the maximum diaphragm excursion in the elderly as observed on ultrasound.

ABSTRACT

@#Objective To analyze the feasibility of six-minute walk test (6MWT) before pulmonary lobectomy and prediction for postoperative outcome. Methods A total of 580 patients who were hospitalized in the department of lung surgery from May, 2017 to May, 2019 were reviewed, and 274 eligible patients were selected, who underwent first surgery and the surgical method was pulmonary lobectomy. They were divided into two groups based on the results of 6MWT before operation. The cut-off value of six-minute walk distance (6MWD) was obtained by receiver operating characteristic curve (ROC) area under curve (AUC). The postoperative outcome and the occurrence of cardiopulmonary complications in the two groups were analyzed. Results Compared to patients with 6MWD > 449 meters, the age was significantly older (P < 0.001), the forced expiratory volume in the first second (FEV1) was poor in patients with 6MWD ≤ 449 meters (P < 0.05), and other factors such as surgical resection site, pathological stage, gender, etc., were not significantly different (P > 0.05). The incidence of postoperative cardiopulmonary complications was significantly higher (OR = 2.672, 95%CI 1.488 to 4.798, P = 0.002), and the postoperative extubation time and hospital stay was longer in patients with 6MWD ≤ 449 meters than in patients with 6MWD > 449 meters (P < 0.05). 6MWD ≤ 449 meters was an independent risk factor for postoperative cardiopulmonary complications (OR = 2.395, 95%CI 1.299 to 4.415, P = 0.005). Conclusion As a simple function test, 6MWT can be routinely used to assess the physiological function of patients undergoing pulmonary lobectomy. Patients with 6MWD ≤ 449 meters may be in higher risks of postoperative cardiopulmonary complications.

ABSTRACT

Objective:To observe the relationship of ventilatory efficiency to cardiac function, especially heart rate recovery after exercise for patients with chronic obstructive pulmonary disease (COPD). Methods:From January, 2019 to December, 2020, 190 patients with COPD were recruited for Cardiopulmonary Exercise Testing. The general condition, medical history and medication history, lung function test and parameters of Cardiopulmonary Exercise Testing were recorded. They were divided into normal group and delay group according to whether the heart rate decline more than twelve beats within a minute after Cardiopulmonary Exercise Testing. Results:There were 89 patients (46.84%) in the delay group. Compared with the normal group, the delay group were older (Z = 2.282, P < 0.05), with less ratio of force exiratory volume in the first second in prediction (FEV1.0%) (Z = 3.626, P < 0.001), maximum power (t = 5.547, P < 0.001), breath reserve (BR) (t = 2.122, P < 0.05) and higher minimum ventilation equivalent of carbon dioxide (VE/VCO2nadir) (Z = 3.296, P = 0.001). Logistic regression showed that the COPD severity, VE/VCO2nadir and BR correlated with heart rate recovery. After adjusting for gender, age, body mass index and COPD severity, VE/VCO2nadir was an independent risk factor for delayed heart rate recovery (OR = 1.203, 95%CI 1.032 to 1.873, P = 0.004), and the best cut-off point was 33.15 (AUC = 0.6387, 95%CI 0.5595 to 0.7178, P = 0.001). Conclusion:The ventilatory inefficiency may increase the risk of abnormal heart rate recovery after exercise in COPD patients.

ABSTRACT

Acute myocardial infarction (AMI) is less frequent in young individuals (≤ 45 years) than in older ones (> 45 years). Young AMI patients differ from older AMI patients in different ways. This article aims to assess the differences between young and older AMI patients. A search was made in the database of Cochrane Library, PubMed, BioMed Central and Embase, sence their establishment to December 2016, using the key words: risk factors, clinical characteristics, acute myocardial infarction and young. Meta-analysis was performed by using the Review Manager 5.3 software, pooled odds ratios and 95% confidence intervals were used to assess the strength of differences. Eight studies with fairly quality, enrolling 13,358 patients in the analysis. Compared with older AMI patients, young AMI patients had a higher rate of smoking and obesity (OR = 2.71,95%CI:1.87 to 3.92; OR = 1.76,95%CI:1.13 to 2.74), higher rate of family history of coronary artery disease and alcohol consumption (OR = 2.36,95%CI:1.22 to 4.59; OR = 1.76,95%CI:1.04 to 2.97). Moreover, Young AMI patients had a lower rate of hypertension and diabetes mellitus (OR = 0.52,95%CI:0.37 to 0.73; OR = 0.58,95%CI:0.50 to 0.67). No significant differences were observed in hyperlipidemia, a subgroup data-analysis showed a higher total cholesterol, triglyceride lipase, and low-density lipoprotein levels (p < 0.05), and lower levels of high-density lipoprotein (p < 0.01) in young AMI patients. Smoking, family history of coronary artery disease, obesity and alcohol consumption are the most main risk factors of AMI among young individuals, and young AMI patients have better prognosis than older ones

ABSTRACT

Esophageal cancer is the most common malignant tumor of the digestive tract in China. At present, surgical resection is the main method to cure esophageal cancer. With the population and implementation of the enhanced recovery after surgery, perioperative physical therapy plays a more and more important role. This review discussed the necessity of preoperative cardiopulmonary function assessment in patients with esophageal cancer, the effects of preoperative respiratory training and aerobic exercise on optimizing physical fitness and reducing postoperative complications, and the content of early postoperative physical therapy. Preoperative cardiopulmonary function of esophageal cancer patients is not strongly related with the outcome, which may be related to the complexity of esophageal cancer surgery and the high incidence of postoperative complications. Preoperative inspiratory muscle exercise can improve the pulmonary function of patients to some extent, but still lack of large sample size research. Preoperative aerobic exercise for esophageal cancer can improve the patient's physical reserve and should be valued. Postoperative physiotherapy priorities includes pain management, chest physical therapy and early mobilization. Exercise prescriptions such as intensity and time for early mobilization are yet to be further studied.

ABSTRACT

Objective To elucidate the inhibitory effect of 131I-fulvestrant on the growth of human breast cancer cells and the effect on the important organs. Methods MTT assay was used to clarify the difference in killing effects of the 131I-fulvestranton on MCF-7 cells and MDA-MB-231 cells.Breast cancer MCF-7 cell xenografts in nude mice was establishied, and two different administration methods of the 131I-fulvestrant in the MCF-7 cell to nude mice were given respectively.Organs and tumours of nude mice were observed. Results MTT assay demonstrated that 131I-fulvestrant had similar cytotoxicity against MCF-7 cells and MDA-MB-231 cells, and the former was slightly stronger.Transient contact experiments showed that 131I-fulvestrant could play a tumor suppressor effect on MCF-7 cells continually, but MDA-MB-231 cells wasn't.After the injection of 131I-fulvestrant via caudal vein, the radioactivity concentration on tumor site accounted for (4.33 ± 0.28)% of the total injection, and the volume of the tumor reduced before gradually increasing again.Radioactivity in the blood accounted for (20.76 ± 2.54)% of the total injection.Qrgans like liver and kidney also showed radioaction distribution. Its distribution was accorded with the distribution of estrogen receptor.Local injection of 131I-fulvestrant got powerful killing effect on the tumor, and the distribution of the radioaction was mainly confined within the tumor. Conclusion 131I-fulvestrant has a good inhibitory effect on MCF-7 breast cancer cells, which is a superposition of radiotherapy and endocrine therapy, and it is controllable on the general condition and important organs of nude mice.

ABSTRACT

To explore the effect of Kudiezi injection on renal function in the real world, in order to provide the basis for the clinical medication safety. Patient aged between 18-80 were selected from 18 large hospitals information system (HIS) databases established by clinical research institute for basic traditional Chinese medicine of China academy of Chinese medical sciences. The patients who were treated with Kudiezi injection (24 225 cases) were defined as the exposed group, whereas those who were not treated with Kudiezi injection (14,191 cases) were defined as the non-exposed group. The propensity score method was used to balance the confounding factors. Classic logistic regression, GBM weighted propensity score logistic regression, GBM propensity score weighted logistic regression with covariate and sensitivity analysis were adopted to study the effect of Kudiezi injection on renal function. The results showed no significant difference in the possibility in abnormality in serum creatinine (Scr) (P = 0.940, 0.679, 0.834) and urea nitrogen (BUN) (P = 0, 0.045, 0.164) between both groups. Therefore, the existing data indicated no damage of Kudiezi injection on renal function. Because this study is a retrospective study based on the real world, there may be unknown confounding factors and potential bias. Therefore, further studies shall be conducted to monitor whether Kudiezi injection causes damage on renal function, in order to ensure the clinical medication safety.

ABSTRACT

Objective To study the relationship between the constant work rate exercises and dynamic hyperinflation in patients with stable COPD. Methods Routine lung ventilation function and 80%Wmax constant power bicycle ergometer exercises were tested in 30 stable COPD patients. We analyzed the correlation coefficient of Time of tolerant exercise , degree of dyspnea , VO2 max%, VO2 @kgmax% and IC%. Results All COPD patients existed varying degrees of exercise capacity accompaning with dynamic hyperinflation in the bicycle ergometer exercises. By Spearman correlation analysis , the VO2 @kg% of constant power exercises in patients with COPD is correlated with the DH% (r = -0.480, P = 0.007) and ICq% (r = 0.667, P = 0.01). Conclusion Dynamic hyperinflation is probably the prominent factors for the capability of constant work rate exercises in patients with stable COPD. Inhibition of dynamic hyperinflation may help improve exercise capability in patients with constant power exercise.

Objective To explore the correlation of the throid function and malnutrition in maintenance hemodialysis(MHD) patients. Methods Fifty-four MHD patients and 16 normal controls were enrolled as our subjects and named as MHD group and control group.Serum concentrations of total triiodothyronine (TT3),total thyroxine (TT4),free thyroine (FT4),free triiodothyronine (FT3),and thyroid-stimulating hormone (TSH) were measured by enhanced chemiluminescence immunoassay.Their anthropometry, biochemical assays,malnutrition-inflammation score (MlS) and micro-inflammation parameters such as high sensitivity C-reactiveprotein(hs-CRP) were also measured. Results Serum T3,FT3,T4,FT4 concentration in MHD patients were (1.18 ±0.31) nmol/L,(2.61 ±0.62) pmol/L,(100.00 ± 19.96) nmol/L,(8.56 ±0.85) pmol/L respectively, lower than that of control group ((2.95 ± 0.27) nmol/L,(7.12 ± 0.94) pmol/L,(136.25 ± 19.68) nmol/L,(8.89 ± 0.56) pmol/L respectively,P < 0.01).while there was no significant change on TSH (P =0.058).According to albumin nutrient grading standard:the morbidity of malnutrition was 75% (39/54).The morbidity of malnutrition was 100% based on the criteria of MIS.Of which, mild malnutrition rate was 29.63% (16/54),moderate rate for 37.04% (20/54) and sever rate for 33% (18/54).The levels of serum creatinine, hemoglobin, serum iron, upper arm circumference, body mass index(BMI),deltoid skinfold,hs-CRP,T3,FT3,T4,FT4,TSH in MHD patients with mild, moderate and serve rate MIS were significant different(P < 0.01).Of 54 MHD patients,32 cases were with micro inflammatory state, accounting for 59.26% (32/54).There was a positive correlation between FT3 and albumin (P <0.01).T3 was related to serum creatinine(P < 0.05).Thyroid function parameters showed a negative correlation with hs-CRP,MIS(P < 0.01).And a negative correlation was seen between nutrition indices and hs-CRP and MIS (P < 0.01).Conclusion Thyroid function can reflect the nutrition status of MHD patients to some degree, and both relate with miro-inflammation.Furthermore,MIS is a useful index for the estimation of malnutrition inflammation status.

ABSTRACT

Objective To study the effect of electrical stimulation breath training on lung function of patients following pulmonary lobectomy. Methods 62 patients following pulmonary lobectomy were randomly allocated into experimental group (n=30 )and control group (n=32). The experimental group received a 4-week supervised electrical stimulation breath training program using an electric stimulus feedback trainer (20mins per time, 3 times per week); The control group received postoperative routine nursing. Cadiopulmonary function evaluation of 2 groups were tested before and after the experiment. The evaluation included the 6-min walking test (6MWD), FVC, FEV1,W,AT and VO2max/kg. Results After 4 week training, the value of 6MWD,W,FVC,FEV1 all improved, compared to the baseline value (P < 0.05) and the value of 6MWD,W,FVC,FEV1 were more obvious in experimental group, compared to control group(P<0.05). The AT value and the VO2max/kg value increased than the baseline value (P<0.05)and the improvement degree was more remarkable in experimental group than that in control group (P<0.05). Conclusion Electrical stimulation breath training can improve cardiopulmonary function of the patients following pulmonary lobectomy.
